We have now published our first Area Plan DELIVERING CHANGE TOGETHER in which we set out how we will work as a partnership over the next five years to continue the transformation and integration of care and support in our region and address the issues identified in our recent Population Assessment.
In March 2017 we published our first Population Assessment. Required under Section 14 of the Social Services and Wellbeing (Wales) Act, this assessment was carried out jointly by the three local authorities and Hywel Dda University Health Board, with input from users, carers and colleagues in the third and independent sectors.
Section 14A of the Social Services and Well-being (Wales) Act requires us to produce an Area Plan setting out how we will work together to address the findings and recommendations of our Population Assessment.
The Population Needs Assessment provides a comprehensive overview of care and support needs across the region. A range of data relating to the region and the constituent LA areas was used to provide a comprehensive picture of current and future care and support needs and how these are currently being met.
We have developed this tool to make both our area plan and population assessment readily accessible online in a user friendly format. Going forward, the tool will provide information on our progress in delivering the plan. It will also provide access to the key data sets used in our population assessment to understand local need and monitor change.
