West Wales Care Partnership Area Plan and Population Needs Assessment

Section 14A of the Social Services and Well-being (Wales) Act requires us to produce an Area Plan setting out how we will work together to address the findings and recommendations of our Population Assessment.

The Population Needs Assessment provides a comprehensive overview of care and support needs across the region. A range of data relating to the region and the constituent LA areas was used to provide a comprehensive picture of current and future care and support needs and how these are currently being met.

We have developed this tool to make both our area plan and population assessment readily accessible online in a user friendly format. Going forward, the tool will provide information on our progress in delivering the plan. It will also provide access to the key data sets used in our population assessment to understand local need and monitor change.

The West Wales Regional Partnership Board brings together partners from local government, the NHS, third and independent sectors with users and carers with the aim of transforming care and support services in the region. Find out more here
